NEW CLIENTS
Welcome and we look forward to meeting you. All NEW clients must complete a virtual consultation with my Nurse Practitioner, Maria (Step 1). The consultation is done on your phone. Instructions on how to book the consultation are below. Please text Maria, 314-605-9671, after you schedule your virtual consultation if you will be using anything other than an iPhone for the consultation.
If I have never treated you, you must complete Step 1. After Step 1 is complete, you can go to step 2 and book your service with me for a time after your virtual consultation. Please contact me if you have any questions about this process, 815-581-5551.
Step 1 New Clients Only (Current Clients skip ahead to Step 2)
Book a free online consultation with Maria, my Nurse Practitioner. A credit card or debit card is required to hold your appointment. Nothing will be charged unless you are a no-show or cancel less than 24 hours in advance without contacting Maria, 314-605-9671. This consultation is required before treatment can begin.

- Set up your account using a password you will remember. (You will need to sign back into this for your virtual appointment)
- Before you leave the site, fill out the intake forms listed on the left menu.
- Click the Step 2 button and schedule your service
